Terms and Conditions
Minimum one-year membership. Annual enrollment fee for each family member is non refundable. If a patient wishes to void the plan, all completed dental work will convert to regular office fees. Enrollment date begins on the date the enrollment fee is paid and shall expire after 365 days. Non-transferable. All family members must reside in the same household, or be full-time student. Family members cannot substitute in for another family member. Family members can enroll at different times of the year. This is not an insurance product. Nonsurgical periodontal therapy is not included in the Silver Plan, however, it is subject to the 20% discount. Two emergency visits included with plan, $85 per visit thereafter. Cannot be used with any insurance or other offers. All treatments must paid in full to keep plan in effect. All exams, X-rays, and cleanings must occur within the year of enrollment and cannot be carried over to the next year. Annual enrollment fees and fees of procedures are subject to change.
